1. An order letter, containing information for:
   • applicant’s name, citizenship and address/domicile,
   • conventional priority data if any;
2. 4 images, size 7x7 cm;
3. Description of the mark if it is a figurative or combined one;
4. Certified copy of the priority application if any priority claimed;
5. List of goods/services in which the mark is to be registered;
6. Classes of goods/services according to the International Classification;
7. Signed and sealed form of power of attorney;
8. For collective or certificate trademark the rules for its using has to be attached.
